Terms & Conditions
Rescheduling and Cancelation Policy
To ensure that all of our clients are treated fairly and are able to receive treatment in a timely manner, Urban Unwind Wellness asks that you provide at least 24 hours notice if you need to change, reschedule, or cancel your appointment. If you cancel or reschedule your appointment without providing 24 hours advance notice or if you fail to keep your appointment (i.e. no-show), your credit card will be charged a fee of $50.00. Credit card verification is required at time of scheduling.
Payment Policy
For massage packages, subscriptions, and gift cards/certificates, payment is accepted immediately upon placing your order. For subscriptions, payment occurs automatically each month via the credit card you place on file until you choose to cancel your subscription. For all other ordinary, single massage appointments, credit card verification is required at time of scheduling, but payment is accepted after completion of the session. Note that all credit card processing is handled securely through Square Payment Processing - Urban Unwind Wellness does not personally store any client credit card information and cannot view your credit card information other than the card type and the last four digits of the card number.
When ordering a monthly subscription, you acknowledge and understand the subscription entitles you to the number of massages listed per month and that those massages must be scheduled within one month of billing - there is no rollover or accumulation of unused monthly massage sessions. Subscriptions and packages may only be used by the named purchaser and are not transferrable to other persons. The coupon code issued at the time of purchase of a subscription or package must be applied at time of scheduling in order to be valid - appointments scheduled without application of a coupon code will be treated as a conventional single massage appointment and will be subject to the regular session fee at the completion of the massage.
Comfort, Communication, & Understanding of Risk
You confirm that if you experience any pain or discomfort, you will immediately inform the therapist so that pressure or techniques being used can be adjusted to your comfort level. You agree you will not hold the massage therapist responsible for any pain or discomfort you experience during or after the session. Although massage therapy is a very low-risk therapy, you understand the common risks associated with massage therapy which include, but are not limited to: superficial bruising, short-term muscle soreness, and exacerbation of undiscovered injury.
Massage Therapy Scope of Practice
You confirm you understand that massage therapy is not a substitute for a medical examination or treatment and that you should see a physician or other qualified health specialist for any mental or physical ailment of which you are aware. You also understand that massage therapists do not diagnose illness or disease. Although suggestions or recommendations may be made based on the therapist's experience and success with other clients, nothing said by the massage therapist during the massage should be construed as diagnosis.
Results & Expectations
Each person reacts differently to massage therapy. Although most clients experience positive outcomes from massage treatment, results and outcomes are not guaranteed from any single treatment or series of treatments.
Disclosure of Conditions & Sensory Deficiencies
You confirm that you do not have any injuries or conditions that prevent you from receiving massage therapy. You understand the importance of informing the massage therapist of all medical conditions and medications that you are taking and that there may be additional risks based on your physical condition. You also confirm you do not currently have diminished sensory deficiencies that decrease the normal ability to detect pressure and pain. This includes causes from both physiological conditions (e.g. neuropathy or nerve damage) and medicinal agents (e.g. over-the-counter analgesics, muscle relaxers, opiods, and marijauna).
Client Intake Discussion & Privacy
During your first appointment as a new client, we will spend several minutes discussing your current health and treatments needs. This an essential part of creating a safe treatment plan for you. Please be aware this step cannot be skipped and constitutes part of your initial session time so actual hands-on time will be a few minutes shorter than a typical session. Also, all client personal information gathered during this initial intake discussion is treated as private and confidential. This information is used solely by Urban Unwind Wellness for treatment planning and client communication. It will not be disclosed to other parties. During the intake discussion, you will also be given the opportunity to ask questions about massage therapy and will receive answers to those questions before beginning the session.
Tardiness
Appointment times are as scheduled and cannot extend beyond the scheduled appointment timeframe. If you are late, you are still welcome to the session, but you will be limited to the time remaining and will be required to pay the full session fee.
Sickness
Massage and bodywork are not appropriate care for or during contagious illnesses. Please cancel your appointment as soon as you are aware of an infectious condition. If you arrive for a session and you are a contagion risk, you will be turned away and asked to reschedule. (Note: Appointments cancelled without providing at least 24 hours advance notice are still subject to a $50.00 cancelation fee.)
Informed Consent and Right to Refuse Treatment
By agreeing to these Terms of Service, you confirm you are voluntarily requesting and consenting to receive massage therapy. You also understand that you or the massage therapist may terminate the session at any time for any reason. You agree your consent is informed and voluntary, and you understand that you may withdraw your consent at any time except for actions already taken.
